Mass General Brigham’s Care Continuum (CC) Team is pleased to announce the launch of our Population Health Management (PHM) CC iSolve Grant for 2022. The goal is to support early stage, RSO-based projects aimed at optimizing SNF utilization through qualitative QI efforts, human-centered design, or implementing an intervention with a promising solution. Each awardee will be provided mentored project implementation and evaluation from the PHM CC team and Springboard. Additionally, all selected projects will be awarded up to $50,000.

To align with our overall program goals, proposals focusing on the following areas will be considered:
